Privacy Policy

This policy explains how Incentive Care LLC ("Incentive Care," "we," "us") handles information collected through incentivecare-llc.com (the "site"). The site is intended for physicians, practice staff, health plans and business partners. It is not intended for patients or Medicare beneficiaries.
Information you give us
When you submit the "Request a Conversation" form, we collect the information you enter:
- your name, practice name, provider type, number of providers and county;
- the Medicare Advantage plans your practice accepts;
- your email address and phone number; and
- anything you write in the message box.
Please don't include patient information. The form is not designed to receive protected health information. If patient information is sent to us through the site, we will delete it and will not use it.
Information collected automatically
- Server logs. Like most websites, our hosting provider records technical information such as your IP address, browser type, the pages requested and the date and time, to operate and secure the site.
- Campaign tags. If you arrive from a link or QR code that includes campaign tags (for example
utm_source=conference), the site includes those tags and the page you landed on with your form submission, so we know which outreach reached you. These tags are held in your browser's session storage and are cleared when you close the tab. - Spam protection. After you send the form, our web server keeps a scrambled (salted and hashed) form of your IP address, together with the time you sent it, to limit repeat or automated submissions. Records more than an hour old are deleted automatically the next time the form is used.
The site does not set cookies, does not use advertising or social-media tracking pixels, and does not sell or share your information for advertising.
How we use information
- to respond to your inquiry and talk with you about joining the network;
- to support credentialing, contracting and onboarding if your practice decides to join;
- to understand which outreach efforts bring practices to us; and
- to keep the site secure and working properly.
How we share information
Form submissions go to our Network Development team by email. We use service providers to host the site and deliver email, and they may process information on our behalf. We may also disclose information if required by law or to protect our rights. We do not sell personal information.
How long we keep it
We keep inquiry information as long as needed to respond to you and to maintain our business relationship, and as required by law. You may ask us to delete your information at any time.
Security
The site is served over an encrypted (HTTPS) connection. No method of transmission or storage is completely secure, but we take reasonable steps to protect the information you share with us.
Children
The site is not directed to children under 13, and we do not knowingly collect information from them.
