These terms apply to your use of incentivecare-llc.com (the "site"), operated by Incentive Care LLC ("Incentive Care," "we," "us"). By using the site, you agree to these terms.

About the site

The site provides general information for physicians, practices, health plans and business partners about Incentive Care, a network of independent physicians. It is not medical advice, does not create a provider–patient relationship, and is not an offer of insurance or health plan coverage.

Incentive Care is not a health plan

Incentive Care does not offer, sell or enroll anyone in health plans. Plan names appear on the site only to describe the network's relationships with those plans. For questions about coverage or benefits, patients should contact their health plan.

Network participation

Information on the site is not a contract. Participation in the network, and in any health plan through the network, is subject to credentialing, a signed agreement with Incentive Care, and each plan's own requirements. Submitting the contact form does not guarantee participation.

Your submissions

Please provide accurate information and do not submit patient or other protected health information through the site. You agree not to use the site to send spam, to interfere with its operation, or for any unlawful purpose.

Intellectual property

The site's content and the incentiveCare name and logo belong to Incentive Care. Health plan names are the property of their respective owners; their appearance on the site does not imply endorsement of the site.

Accuracy and links

We try to keep the site accurate and current, but it is provided "as is," without warranties of any kind. Links to other websites are provided for convenience; we are not responsible for their content.

Limitation of liability

To the fullest extent permitted by law, Incentive Care is not liable for any indirect, incidental or consequential damages arising from your use of the site.

Governing law

These terms are governed by the laws of the State of Florida, without regard to its conflict-of-law rules.
